Shelley Basinger: I’m here with Joseph Park from Universal Health Corporation and, Joseph, we’re talking about transitioning from a hospital to somewhere else. Maybe it’s home, that’s what we want, but other times we need to put some steps down in our care. What are patients’ options when they leave the hospital?

Joseph Park: Well, you can still go home, but it may require home health or a private caregiver. There’s also skilled nursing facility placement or in-patient rehab, and then the other option would be long-term care.

Shelley Basinger: Let’s talk about the transition from hospital to a skilled nursing facility. What types of changes would a person expect to see if they’re receiving that type of care?

Joseph Park: People need to realize you’re stepping down, so the level of care is going to change and the possibilities of a life transition are very real. In a skilled nursing facility, the emphasis will be put on rehab, and patients’ involvement in their rehab will be what determines if they can go home.
